For hospice intake teams and community marketers

Is this referral ready for hospice?

Take a photo of the hospital notes. In a couple of minutes, Ready for Hospice tells you if the patient is eligible as is, or exactly what to ask to find out.

Built on Medicare's hospice guidelines (Palmetto GBA). Decision support: your physician still certifies.

Ready for Hospice
Eligibility Not yet: ask these questions

May qualify under the dementia guidelines, but the notes don't show FAST stage 7 yet.

One job, done well

No new system to learn. Snap the notes, read the answer, make the call.

1

Snap or upload

Take photos of the referral's notes on your phone, one per page, or upload the PDF packet. Pasting text works too.

2

Checked against Medicare

The notes are read against the Medicare hospice guidelines (LCDs) for the patient's likely hospice diagnosis, criterion by criterion.

3

Eligible, or what to ask

You see "Eligible as is", or the questions to ask the referral source, the family or the physician to find out.

Everything intake needs, on one screen

Print it, or hand it to the admission nurse.

  • The answer up front: eligible as is, not yet, or not eligible now, in a sentence a marketer can repeat.
  • Questions to ask, grouped by who to ask, each with the gap it closes.
  • The likely hospice diagnosis and its ICD-10 code, with coding cautions.
  • Each Medicare criterion marked met, not met or unknown, with where it was found in the notes.
  • Red flags an auditor would question, like copied-forward notes or a patient choosing rehab.
  • What to document, split by physician, admission nurse, marketer and front desk.
  • A team dashboard: every referral tracked to admit or decline, monthly usage, and an email to your physicians when a review is ready.

Honest questions, not leading ones

Most referrals that miss on paper are missing documentation, not eligibility. Ready for Hospice helps your team find out what's true, and keeps you on the right side of an audit.

Every question is open and factual. It never suggests the answer that would qualify the patient.

✓ "How much does she eat in a day now?"
✕ "Would you say she eats less than half her meals?"

Patient records stay protected

Under a BAA

We sign a business associate agreement with your hospice. Records are reviewed by AI running inside our Amazon Web Services account, which is covered by Amazon's BAA.

Questions

Does Ready for Hospice decide who is eligible?

No. It's decision support for your intake team. The hospice physician decides and certifies terminal illness, in their own words. Patients outside the guidelines can still qualify on an individual basis.

Which Medicare guidelines does it use?

The hospice Local Coverage Determinations from Palmetto GBA, the Medicare contractor for Georgia hospices: heart and lung, dementia, neurological, kidney, liver and adult failure to thrive. Cancer uses another contractor's guidelines for reference. Support for other contractors is coming.

Can it read handwritten notes?

Clear photos of printed notes work best. Legible handwriting usually works. If something can't be read, the review says what's missing.

How long does a review take?

Usually a minute or two. A full hospital packet can take a few minutes.

Do you keep our patients' records?

The notes and photos you send are used for the review and never saved. The review itself (labelled with the patient's initials and referral date, not their name) is saved, encrypted, for 12 months, so your team can track the referral to admit or decline. Only your hospice's staff can see it. It's deleted automatically after 12 months, or sooner if you delete it.
