Draft for legal review: not yet in effect.
Privacy policy
Ready for Hospice is software that hospice intake teams and community marketers use to check whether a referred patient appears to meet Medicare's hospice guidelines. Staff send a referral's notes (photos, PDFs or pasted text) and get back a review. This policy explains what information Ready for Hospice handles, why, and who can see it.
Who is responsible for your information
The hospice that uses Ready for Hospice decides which records are sent and creates every account. For patient health information, the hospice is the covered entity under HIPAA and Healthcare Henry LLC acts as its business associate under a signed business associate agreement. If you have a question about a patient's record, contact the hospice first. You can also contact us at the address below.
What we handle
| About | What | Why |
|---|---|---|
| Patients | The notes, photos and PDFs a staff member sends, and the review that comes back, labelled with the patient's initials and referral date. | To produce the eligibility review and let the hospice track the referral. Notes, photos and PDFs are used for the review and never saved; the review is saved, encrypted, for 12 months. |
| Hospice staff | Name, email, the hospice they work for, sign-in activity, and how many reviews they've run. | To sign people in, keep each hospice's account separate, and count reviews for billing. |
We do not use advertising, ad tracking or third-party analytics, and we never sell information.
How reviews are made
Records are reviewed by an AI model (Claude, made by Anthropic) running in Amazon Bedrock inside our Amazon Web Services account, which is covered by Amazon's business associate agreement. Records are not used to train AI models. The review is decision support: the hospice physician decides and certifies terminal illness.
Who can see it
- Saved reviews can be seen only by signed-in staff of the hospice that ran them. A shared link opens only after sign-in. Staff may print a review for use inside their hospice.
- When a review is ready, people the hospice chooses get an email with who ran it, the answer and a link. The email contains no patient details.
- No hospice can see another hospice's information.
- We share information with service providers only to run Ready for Hospice (for example, secure hosting), each under a business associate agreement, or when the law requires it.
How long we keep it
Notes, photos and PDFs are never saved after the review. Reviews are kept, encrypted, for 12 months and then deleted automatically; a hospice admin or the person who ran a review can delete it sooner. Nothing is stored on the phone. Staff account information is kept while the hospice uses Ready for Hospice. When a hospice stops, we delete its account information as set out in its agreement with us.
Security
Information is encrypted in transit and at rest. Every person signs in with their own account and an authenticator code, each hospice's information is kept separate, and opening, changing or deleting a saved review is recorded (who and when, not what's in it).
Your choices
To see, correct or delete your account information, or to close your account, contact your hospice or email us.
Children
Ready for Hospice is for hospice staff and is not meant for children.
